Science now suggests that the brain itself can be retrained to reduce — and often eliminate — chronic pain.

Chronic back pain (CBP) is a leading cause of disability, and treatment often doesn’t work. Approximately 85% of cases are primary CBP, patients suffer from a wide variety of symptoms:
Fear
Social avoidance
Anxiety and depression
As someone who suffered from chronic back pain, I know these symptoms all too well.
Here’s why this data matters…
Yoni K. Ashar, Tor D. Wager, along with other leading researchers stated:
“Chronic back pain affects 20% of people with an estimated annual cost of over $600 billion. About 85% of chronic back pain cases are "primary" - meaning no definitive physical cause can be identified in the spine or surrounding tissues. Traditional treatments often show limited effectiveness for pain reduction, even when they improve function.”
Let me explain…
Traditional treatments that target the body alone often fail to reduce this type of pain.
So what did the study test?
A therapy called Pain Reprocessing Therapy (PRT). The approach helps patients reframe their pain as a “false alarm” coming from the brain, not from injury.
Here’s what happened…
151 participants with chronic back pain (lasting 6+ months, average 10 years duration)
Three groups: Pain Reprocessing Therapy, open-label placebo injection, and usual care
Participants had brain scans (fMRI) and were followed for 1 year
Enter Pain Reprocessing Therapy (PRT):
PRT involved 8 one-hour therapy sessions over a month plus one physician consultation.
The approach was to help patients rethink their pain as brain generated signals rather than tissue damage, using:
Exposure exercises to feared activities
Techniques addressing emotional factors
Guided reappraisal of pain sensations during movement:
Personalized evidence that their pain was centralized (brain-based)
The results destroyed skepticism within the scientific community:
Pain reduction: PRT group averaged 1.18/10 pain vs 2.84/10 (placebo) and 3.13/10 (usual care)
Pain-free rates: 66% of PRT participants became pain-free or nearly pain-free vs 20% (placebo) and 10% (usual care)
Durability: Large effects maintained at 1-year follow-up
Brain changes: Reduced pain-related brain activity in regions associated with threat detection and emotional processing
The lesson is simple:
When patients stop believing their pain means “damage,” the brain stops producing so much pain.
